Corporate Membership

Our Corporate Team will arrange for your company’s employees to receive a special “Discounted Corporate Rate” that is not available to the general public.

The Corporate savings, with or without company subsidy, will provide tremendous savings and benefits for both employers and employees alike.
 
To support your Health & Wellness Corporate program, we offer:
- A personalized corporate package and templates to convey information to your employees
- An appointed team member to communicate and personalize the administration of each corporate account
- Corporate membership information (available upon request by company H/R)
 
A membership that is given to an individual at a discounted rate formed from the same group of people and any associations who work under the same corporate facility.
  
 
Corporate Memberships are for 1 year 
Corporate Memberships cannot be transferred or sold
Corporate rate also extends to the employee’s immediate family members that live in the same household (proof of residency is required)
Any employees who are currently members with Shapes will qualify for rate
Members get “full” use of facility 
Memberships are valid at all Winnipeg, Manitoba - Shapes Fitness Centres
location

I.P.F.A. (International Physical Fitness Association) approved.

Payment Date is every 2nd Thursday beginning from the 2nd Thursday of your renewal date. Please Indicate which Corporation you are a part of and any other comments *

By submitting this request, you agree to abide by all the terms and conditions of this membership agreement. You authorize Shapes Fitness Centres to withdraw membership payments as per your corporations membership fee payment information provided by you.
